Assessment and Planning

  • Fill Out a New Client Intake Form: Before the initial assessments, you will be asked to complete a new client intake form. This form helps us gather essential information about your child's needs, background, and specific areas of support to ensure that we are fully prepared for the initial assessment sessions.

  • Initial Assessment: We conduct two initial assessments to better understand the specific needs of your child:

    • In-Clinic/In-Home Assessment: Allows us to observe your child in a controlled or familiar environment.

    • In-School/Preschool/Community Assessment: Helps us understand how your child interacts in social and educational settings.

  • Program Design & Planning Meeting: Following the assessments, we'll schedule a meeting to go over the proposed support plan and discuss the goals tailored to your child's needs.
